What are the requirements


For Sand and Gravel, Earth filling, Small Scale Mining, and Gratuitous and Guano Permits

Preliminary Requirements:

  1. Application Letter address to the City Mayor thru the City ENRO
  2. Sketch Plan (Prepared and Signed by Deputized Geodetic Engineers of MGB)
  3. City Planning Certification (Zoning)
  4. Barangay Resolution (Favorably recommending the application of Commercial, Industrial, Exclusive Sand and Gravel, Earth fill, Small Scale Mining, Private and Government Gratuitous and Guano Permit) (Submit two Barangay Resolutions of concerned barangays, if river is the common boundary of each barangay)
  5. Certified True Copy of Land Title from the Register of Deeds or Deed of Sale.
  6. Certificate of No Encumbrance (certifying the Land Title of not being mortgage)
  7. Agreement between the Lot owner (if in case, the applicant is not the lot owner of the applied site)
  8. Special Power of Attorney (SPA) for applicant being represented / Board Resolution for Corporation / Association or Partnership or Cooperative application.
  9. Two Valid ID’s (SSS, Professional License, TIN/PhilHealth ID with picture, etc.)
  10. Latest bill (water, electric, telephone, etc.)
  11. Three (3) Long Folders with fastener
  12. Official Receipt from City Treasurer’s Office for: a. Area Verification Fee – P150.00 b. Filing & Processing Fee – 50.00 ———- P200.00

Final Requirements:

  1. Application form duly accomplished and notarized (MGB Form No. B-3, B-3A, B-4. B-4A)
  2. Area Verification report.
  3. Area Clearance from the concerned government agencies that may affect application
    1. City Engineer’s Office
    2. Department of Public Works and Highways
    3. Department of Agriculture (For Earthfill and Quarry only)
    4. National Irrigation Administration (For Sand and Gravel only)
    5. National Commission on Indigenous Peoples (if applicable)
    6. Davao City Water District (Sand and Gravel only)
    7. Other Certification (if necessary)
  4. Certification from MGB-DENR RXI, City ENRO and concerned barangay that Notice of Posting has been complied.
  5. Work Program duly prepared, signed and sealed by a License Mining Engineer or Geologist
  6. Environmental Protection and Enhancement Program (EPEP) for Industrial Application Permit only
  7. Proof of Financial Capability (Bank Account or Income Tax Return)
  8. Environmental Compliance Certificate (ECC) issued by EMB-DENR RXI with Resources Sustainability and Geo-Hazard Assessment (RSGA)
  9. Certified True Copy of duly registered Article of Partnership/Corporation/Association (SEC Registration), (if Corporate application)
  10. Surety Bond in the amount of P20,000.00 (duly notarized)
Solid Waste Management Certificate

For Preliminary Approval and Locational Clearance (PALC) and High Risk Business Establishments

1. Submit request letter for SWM Certificate addressed to:

        • ENGR. MARIVIC L. REYES
        • Acting Department Head
        • City Environment and Natural Resources Office
        • Ma-a, Davao City

2. Attend SWM orientation (RA 9003 & CO 0361-10) at Peoples’ Park, Davao City Schedule of orientation: Monday to Friday (8:00 a.m. – 4:30 p.m.(Attendance Slip will be issued after the orientation.)

3. Prepare your company/business SWM Plans, Programs and Practices in two parts: construction phase and  operation phase. Submit the same along with your Attendance Slip.

          • Prepare your company/business SWM Plans, Programs and Practices in two parts: construction phase and  operation phase. Submit the same along with your Attendance Slip.

b.) COMMITMENT 

                     Discuss the willingness and commitment to implement proper SWM                         based on the existing laws and ordinances.

c.) OPERATIONAL BACKGROUND

                    Say something about the operation of the establishment that includes                      site development, peration proper, raw materials used, production                           result, wastes that will be generated in the process/operation,                                     potential environmental effects and its mitigation, etc.

d.)  SOLID WASTE MANAGEMENT

        • Identify and discuss area sources of SW during construction phase and operation phase
        • Identify types of solid waste generated
        • Methods of segregation ( identify strategic locations for garbage bins)
        • Provision of four (4) types of segregator bins with proper label and closing lids
        • Methods of storage ( MRF / buy back center )
        • Methods of disposal
        • Presence of composting facility for biodegradable wastes
        • Make a flow chart

 e.)  OTHER WASTE & NUISANCE MANAGEMENT (if applicable)        

        • Wastewater
        • Smoke
        • Dust
        • Odor
        • Noise

4. Ask Order of Payment from this Office and pay the corresponding amount             of ₱500.00 as certification fee at any City Treasurer’s Collecting Offices.                Present receipt upon claiming.        

Note: Two (2) sets of Solid Waste Management Plans, Programs, and Practices shall be submitted to CENRO.

Cutting/Trim/Prune Permit

1. Submit request letter addressed to:

        • ENGR. MARIVIC L. REYES
        • Acting Department Head
        • City Environment and Natural Resources Office
        • Ma-a, Davao City

2. Barangay Certification certifying ownership of trees and no objection to cut/trim/ prune.

3. Photocopy of Land Title/Deed of Donation.

4. SPA/Authorization from the property owner to the authorized representative.

5. Development plan with Sketch Map (if applicable).

6. PTA/Barangay Resolution.
